Patient Forms
Preparing for your procedure includes following the preoperative instructions Dr. Quatela, Dr. Lee, and Dr. Montague provide, completing the necessary forms and, if needed, arranging for financing.
First, please complete the Patient Demographic Sheet below and bring it with you to your initial consultation appointment, along with a photo ID and insurance card. Although hair restoration procedures are not covered by insurance, our office is required to have a copy on file in case of an emergency.
